Responsibilities include assessment & intervention for barriers to care, acute safety concerns, transition-to-adulthood planning, patient and family coping/stress, and general support as families navigate the healthcare and community systems. As the SW Clinic Leader, they are an active member of the CP Leadership Team, supporting QI initiatives and program development.
