GENERAL OVERVIEW:

This role involves collaborating with an interdisciplinary team of providers, clinicians, health plans, and external partners to advocate for patients and families/caregivers, coordinating care across the continuum. The incumbent is a vital part of the Care Management team, aiming to improve care quality, patient experience, and population health by addressing social determinants of health that impact wellness.


ESSENTIAL RESPONSIBILITIES

  1. Complete initial and ongoing comprehensive assessments, providing interventions and recommendations after engaging with patients and their caregivers/families. Focus on social determinants of health to ensure successful care coordination. (30%)
  2. Collaborate to create safe transition plans for patients (Inpatient/Observation/ED) to ensure timely discharge and appropriate connection with post-discharge care providers and community resources. (25%)
  3. Educate patients, families/caregivers, and physicians about appropriate post-discharge care levels and accessing community support. Advocate through effective communication with team members, payers, and post-acute partners to ensure safe transitions. (20%)
  4. Serve as a resource for counseling and interventions related to treatment decisions and end-of-life issues. Lead discussions to establish care goals and provide crisis interventions for issues such as abuse, violence, mental health, substance use, and surrogate decision-making. (15%)
  5. Promote professional growth through certification, mentoring, and participation in committees. (5%)
  6. Support departmental goals that contribute to organizational success. (5%)
  7. Perform other duties as assigned.


QUALIFICATIONS:

Minimum

  • Master's degree in Social Work (Incumbents in role before 4/21/2025 have 6 months from hire to obtain)
  • Experience in a hospital or healthcare setting
  • LSW and/or LCSW required (Incumbents before 04/21/2025 have 12 months from hire to obtain)
  • Act 34 Criminal Background Clearance
  • Act 33 Child Abuse Clearance
  • Act 73 FBI Fingerprinting Clearance
