Job summary

Intermountain Health is seeking a Social Work Care Manager I to perform psychosocial assessments, develop care plans, and provide counseling with a collaborative care team in Lehi, Utah. The role includes identifying services, coordinating resources, and facilitating smooth transitions of care for patients and families.

The position offers a $500 sign-on bonus for those with 1+ year of relevant experience (external applicants only). Schedule includes days and weekends; FTE is PRN.

Qualifications

• MSW from an accredited program; degree verification required.
• Current state licensure (CSW/LCSW) before or upon completion of supervision hours.
• Basic computer proficiency including word processing and spreadsheets.
• Strong written and verbal communication skills.
• Ability to apply critical thinking and maintain safety standards (BLS when home visits).

Responsibilities

• Assessment & Screening: evaluate patients for transition planning, mental health and care goals.
• Care Coordination: develop and monitor care plans addressing social determinants.
• Behavioral Health Support: use motivational interviewing and therapies to promote mental health.
• Therapeutic Intervention: provide brief individual, group and family therapy.
• Diagnosis & Referrals: identify disorders and connect to services.
• Education & Advocacy: train staff, educate patients, advocate for rights and access.
• Team Collaboration: work with healthcare teams, insurers and community providers.
• Quality & Compliance: lead improvements and ensure policy adherence.
• Advanced Care Planning: facilitate goals of care discussions with patients and families.
