Job summary

Intermountain Health Utah Valley Hospital in Provo, Utah, seeks a Social Work Care Manager I to perform psychosocial assessments, develop care plans, and provide brief therapeutic interventions in collaboration with the care team.

Qualifications

• MSW from an accredited program.
• Current state licensure (CSW/LCSW) required or completed.
• Proficient with MS Office and strong communication skills.

Responsibilities

• Assess patients for transition planning, mental health, substance use, and goals of care.
• Develop and monitor care plans addressing social determinants of health and community resources.
• Provide brief individual, group, and family therapy and psychosocial assessments.
• Identify mental, emotional, and behavioral disorders and connect patients to services.
• Train staff, educate patients, and advocate for care access during transitions.
