Schedule: Full Time; 8:00 am - 4:30 pm, Monday - Friday. Includes 1 full weekend per month (Saturday & Sunday) and a day off during the week.

Location: 601 EAST ALTAMONTE DRIVE, Altamonte Springs, 32701

The role you’ll contribute: The Social Work Care Manager intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs, funding sources, and qualify for community assistance from various programs and agencies. They also assist with transitions of care or discharge planning. Additionally, they offer crisis intervention to patients and families with psychosocial needs and coordinate discharge plans for high-risk patient populations. This role receives referrals from interdisciplinary team members, including physicians, RN Care Managers, staff nurses, and others.

• Psychosocial Assessment and Interventions
• Receives referrals for psychosocial complex needs from the healthcare team
• Provides assessment and reports interventions in child abuse/neglect, domestic violence, adult/elderly abuse, child protection, sexual assault, and human trafficking as appropriate
• Provides consult services for patients lacking decision-making capacity, following guardianship policies and coordinating with Care Management leadership
• Provides consult services for foster care and adoptions

The expertise and experiences you’ll need to succeed:

Minimum qualifications:

• Masters in Social Work (MSW)
• Minimum three (3) years experience in hospital/medical social work

Preferred qualifications:

• Care Management discharge planning experience
• Knowledge of state and federal guidelines pertinent to care management
• Licensed Clinical Social Worker (LCSW)
• ACM/CCM certification
