Job summary

Mercy Hospital Stoddard in Dexter, Missouri is seeking a Social Worker to join the Care Management team to facilitate discharge planning from admission to discharge in collaboration with the healthcare team.

The role may involve intervening with patients with complex or high-risk psychosocial needs while maintaining Mercy's mission and Service Standards.

Qualifications

• Bachelor's in Social Work required for all Social Workers hired after 12/25/2011.
• 0-2 years experience in acute care hospital setting, Minimum Preferred.
• BLS (CPR) at hire date, required, or within 90 days of hire

Job description

The Care Management model provides effective transition planning and length of stay oversight while encompassing quality patient care, patient safety, and financial components; promotes integration of a seamless care model; coordinates patient throughput; encourages healthcare team collaboration and coordination as evidenced by metrics; and optimizes performance and adoption of best practice.

The Social Worker, as part of the Care Management team, facilitates the discharge planning process from admission to discharge in collaboration with the healthcare team. In addition, the Social Worker may intervene with patients who have complex or high-risk psychosocial needs as assigned. The Social Worker performs duties and responsibilities in a manner consistent with the Mercy mission, values, and Service Standards.

Education

Bachelor's in Social Work required for all Social Workers hired after 12/25/2011. (Social Workers hired before December 2011 met previous job description education requirements.)

Certification/Registration

6. Ability to interact effectively with a variety of people and situations at all levels of the organization.
