Job summary

AdventHealth Winter Garden is seeking a dedicated social worker to provide grief counseling, care coordination, and discharge planning across inpatient, observation, and emergency departments. You will assess patients and families holistically to support timely, safe transitions and connections to post-acute services.

This full-time role features four 10-hour shifts with rotating weekends and one late-night shift every other week, at AdventHealth Winter Garden, Florida.

Qualifications

• Master's degree in Social Work required.
• 2+ years of social work experience required.
• 2+ years care management experience preferred.
• Licensure/certification preferred: ACM or CCM.
• Excellent communication and negotiation skills.

Responsibilities

• Provide grief counseling and crisis intervention for patients and families.
• Assess discharge planning needs across inpatient, observation, and ED settings.
• Review medical records and integrate clinical, social, and financial factors into care plans.
• Develop discharge plans and coordinate post-acute care services and community resources.
• Ensure effective communication of care information and continuity of care.

Job Description:

Schedule: Full-Time, Four 10-hour shifts + rotating weekends and 1 late nigh shift every other week.

Shift: 8:30am - 5:00pm, 1 late night 12:30pm-11:00pm

Location: AdventHealth Winter Garden

Primary Job Responsibilities:

• Provides grief counseling, disease adjustment support, crisis intervention, goals of care planning support, and de-escalation services for patients as appropriate.
• Assesses patients’ and families’ wholistically for discharge planning needs in the inpatient, observation and/or emergency departments, including prior functioning, support systems, financial, and psychosocial in a timely fashion to avoid delays in discharge planning.
• Reviews the medical record, including medications, history and physical, labs, and progress notes and incorporates the clinical, social, and financial factors into the transition of care plan.
• Develops discharge plans with appropriate contingency plans throughout the hospital stay to ensure timely care coordination and progression of care, making arrangements for post-acute care services and facilities as well as community care for social needs.
• Leverages technology and follows standard work and best practices to communicate with post-acute care services and facilities to ensure patient care information is communicated for continuity of care, medical records are complete, and discharge reconciliation is accurate.
