Job summary

AdventHealth in Florida is seeking a Care Management leader to oversee daily operations across inpatient, observation, and ED settings, ensuring compliant discharge planning, patient choice, and timely care coordination.

You will lead staff, participate in multidisciplinary rounds, monitor documentation, and partner with hospital leadership to drive throughput and reduce delays, while fostering staff development and a culture of high-quality, compassionate care.

Qualifications

• Master's degree in Social Work is required.
• 3+ direct care management experience is required.
• 2+ experience in social work is required.
• Additional licensure or certification requirements may apply depending on unit/state; consult credential grid for detail

Responsibilities

• Oversees the daily operations of the Care Management department in inpatient, observation, and emergency department settings.
• Ensures documentation and compliance for discharge planning, patient choice, and other regulatory requirements.
• Active participation in daily multidisciplinary rounds to optimize interdisciplinary care coordination.
• Monitors staff documentation, reports, and dashboards to ensure adherence to standard processes and quality care.
• Oversees full discharge planning from admission to discharge to avoid delays and readmissions.
• Collaborates with hospital leadership and executives to elevate pace of care delays and ensure daily progression of care and throughput.
• Identifies process improvement opportunities and participates in interdisciplinary meetings to drive care coordination improvements.
• Manages care management staff escalations of delays and complicated cases to find hospital-wide solutions.
• Educates and reinforces standard work with staff; ensures orientation and competencies are completed.
• Accountable for hiring, onboarding, and retaining staff; ensures a highly productive and engaged team.
• Oversees coordination of family and care team meetings and transitions of care.

Job Description:

• Accountable for hiring, onboarding, and retaining staff, ensuring a highly productive and engagement team.
• Oversees the coordination of family and care team meetings and ensures effective transitions of care and patient and family needs are met.

Education:

• Master's[Required]

Field of Study:

• Master's in Social Work OR Bachelor's in Social Work & Master's in related field

Work Experience:

• 3+ direct care management experience [Required]
• 2+ experience in social work [Required]
