Job summary

The UW Medical Center-Montlake Social Work Department in Seattle is seeking a Social Work Assistant for part-time work (20 hours/week). The role partners with social workers, nurses, patients and families to coordinate care in inpatient and outpatient settings.

You will assist in discharge planning, coordinate resources and referrals, prepare letters for benefits, and provide direct patient care by phone as needed. A background in medical settings and strong multitasking skills are preferred.

Qualifications

  • Bachelor of Social Work or Bachelor's in behavioral sciences with two years as Social Work Assistant I or equivalent.
  • Or Associate of Arts in behavioral sciences with four years of work as Social Work Assistant I or equivalent.
  • Equivalent education/experience may substitute for minimum qualifications except for licensure/registration requirements.

Responsibilities

  • Accept and act on referrals from SW/NCCs in a timely manner, keeping the SW/NCC informed of progress and documenting in the EMR.
  • Assist in identifying, researching and arranging resources for families including transportation, housing, financial assistance, and safety net programs.
  • Prepare letters and documents to assist families in obtaining benefits; copy/fax/mail documents as required; perform other clerical duties.
  • Provide direct patient care by phone, conduct screenings for behavioral health and determine resource needs.
  • Review insurance coverage for mental health benefits to verify eligibility within UW system and community.
  • Assist with safe transition of patient to home or other facilities.

Job Description

The UW Medical Center-Montlake Social Work Department has an outstanding opportunity for a Social Work Assistant.

WORK SCHEDULE
  • Part-Time / 20 hours per week

  • Day Shifts - 2x10 hour shifts on Saturday/Sunday or 2x8 hour shifts on Saturday/Sunday plus every other Monday or Friday

POSITION HIGHLIGHTS
  • Partner with Social Workers (SW), Nurse Care Coordinators (NCCs), patients, their families and the health care team to address patients' progression of care in inpatient and outpatient settings

  • May assist the SW and NCC team in the coordination of the discharge planning process for patients, as well as ongoing coordination of resources and referrals in the outpatient settings

  • This position reports to the Department of Social Work and Care Coordination. Ideal candidates have experience working in a medical setting, demonstrate strong multi-tasking and organizational skills, and work well with a variety of persons and teams.

PRIMARY JOB RESPONSIBILITIES
  • Accept and act on referrals from SW/NCCs in a timely manner, keeping the SW/NCC informed of progress on the case, documenting in the EMR per department standards, and ensuring that handoffs to all parties are clear and timely.

  • Assist the SW/NCCs in identifying, researching, explaining and arranging resources for families. Resources include, but are not limited to, transportation, housing, financial assistance, and safety net programs.

  • Under the direction of the SW/NCC, prepare letters and documents to assist families in obtaining benefits. Copy/fax/mail documents as required. Perform other clerical duties as required.

  • Provide direct patient care by phone, conduct screenings using standard measurement tools for behavioral health and determine specialty care resource needs, dependent on setting and position.

  • Complete review of insurance coverage for mental health benefits to verify and ensure eligibility for services within UW system and community.

  • Assist with safe transition of patient to home or other facilities.

REQUIRED POSITION QUALIFICATIONS

Bachelor of Social Work or a Bachelor's Degree in the behavioral sciences AND two years experience as a Social Work Assistant I or equivalent.

OR

an Associate of Arts Degree in the behavioral sciences AND four years of work experience as a Social Work Assistant I or equivalent.

Equivalent education/experience will substitute for all minimum qualifications except when there are legal requirements, such as a license/certification/registration.
