Job summary

Lutheran Social Services of Wisconsin & Upper Michigan (LSS) is hiring an IRIS Consultant to support adults, older adults, and individuals with disabilities through Wisconsin's IRIS program. This hybrid, full-time role involves coordinating services, building relationships, and promoting self-determination for participants in Milwaukee and surrounding areas.

The IRIS Consultant will handle care coordination, documentation, and collaboration with ADRCs and healthcare providers while maintaining

Qualifications

  • Bachelor's degree or equivalent with supervised experience working with older adults or individuals with disabilities
  • Four (4) or more years of direct experience providing services to older adults or individuals with disabilities
  • Current IRIS Consultant in good standing with a Wisconsin DHS-approved exemption

Responsibilities

  • Develop and maintain trusted relationships with IRIS participants
  • Create and update individualized IRIS Support and Services Plans
  • Conduct monthly contacts and quarterly in-person visits
  • Promote participant choice, self-direction, independence and community inclusion
  • Monitor health, safety and program outcomes
  • Educate about IRIS enrollment requirements, eligibility and resources
  • Collaborate with ADRCs, healthcare providers and service providers
  • Maintain accurate case documentation and timely reporting
  • Assist with annual Long-Term Care Functional Screens
  • Work independently while collaborating with an interdisciplinary team

Lutheran Social Services of Wisconsin & Upper Michigan (LSS) is seeking a compassionate, organized, and self-motivated IRIS Consultant to support adults, older adults, and individuals with disabilities through Wisconsin's IRIS (Include, Respect, I Self-Direct) Program.

This position combines elements of case management, care coordination, social work, disability services, long-term care, and community-based support to help participants maintain independence, achieve personal goals, and direct their own services.

As an IRIS Consultant, you will build meaningful relationships with participants, coordinate services, promote self-determination, and ensure health and safety while supporting participants in living the life they choose.

What You'll Do

As an IRIS Consultant, you will:

  • Develop and maintain trusted relationships with IRIS participants
  • Assist participants in creating and updating individualized IRIS Support and Services Plans
  • Conduct monthly participant contacts and required quarterly in-person visits
  • Promote participant choice, self-direction, independence, and community inclusion
  • Monitor participant health, safety, and program outcomes
  • Educate participants about IRIS enrollment requirements, eligibility, and available resources
  • Collaborate with Aging and Disability Resource Centers (ADRCs), healthcare providers, community organizations, and service providers
  • Maintain accurate and timely case documentation within required systems
  • Complete documentation within established compliance standards and timelines
  • Assist with annual Long-Term Care Functional Screens when applicable
  • Work independently while collaborating with an interdisciplinary support team
Qualifications
Required

One of the following:

  • Bachelor's degree in Social Work, Human Services, Psychology, Counseling, Sociology, Special Education, Nursing, Rehabilitation Services, Healthcare Administration, or a related field and at least one year of supervised experience working with older adults or individuals with disabilities

OR

  • Four (4) or more years of direct experience providing services to older adults or individuals with disabilities

OR

  • Current IRIS Consultant in good standing with a Wisconsin DHS-approved exemption
