Job summary

Genesis HealthCare, Inc. in Charles Town, WV is seeking a licensed social worker to collaborate with the interdisciplinary team, develop plans of care, educate staff, and support patients and families. The role emphasizes promoting patient rights and well-being within a long-term care setting.

Qualifications

• Bachelor's degree in social work or human services field is required.
• Must possess any certifications/licensures as required by State to practice in long-term care.

Responsibilities

Monday through Friday 8am -430pm.

• *Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being. This includes educating staff on the role of social services.
• *Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs.
• *Hold meetings with patients/residents, families, and significant others as requested and to provide support and information to improve quality of life.
• *Perform patient/resident discharge planning activities such as family/patient education, nursing center documentation and make referrals to appropriate agencies and suppliers.
• *Establish relationships and maintain contact and referral flow with community-based agencies/services that benefit our patient/resident population.
