Job summary

Qualifications

Job Summary

Provides social work support to patients in their place of residence. Responsibilities include providing community resources, placement options and counseling to patients and families as needed. Acts as a competent clinician who provides clinical care in a skilled and effective manner, utilizing appropriate technical and decision-making skills.

Job Requirements

Education and Work Experience: Bachelor's Degree or equivalent (some certifications will require Bachelor's): Required

Master's Degree in social work or equivalent combination of education/experience: Required

Three years' experience in a social agency, hospital, or other organization dealing with health and/or welfare: Preferred

Licenses/Certifications

Licensed Clinical Social Worker (LCSW) in state of practice: Preferred

Valid Driver's License (DL) and must be at least 21 years of age or older: Required

Essential Functions

Makes the initial patient assessment as ordered.
