Job Summary:

Provides social work support to patients in their place of residence. Responsibilities include providing community resources, placement options and counseling to patients and families as needed. Acts as a competent clinician who provides clinical care in a skilled and effective manner, utilizing appropriate technical and decision‑making skills.

Job Requirements:

Education and Work Experience:

• Bachelor’s Degree or equivalent (some certifications will require Bachelor’s): Required
• Master’s Degree in social work or equivalent combination of education/experience: Required
• Three years' experience in a social agency, hospital, or other organization dealing with health and/or welfare: Preferred

Licenses/Certifications:

• Licensed Clinical Social Worker (LCSW) in state of practice: Preferred
• Valid Driver's License (DL) and must be at least 21 years of age or older: Required
