Job summary

National Health Foundation seeks a Social Services Manager in Los Angeles to lead the social services team across recuperative care sites. You will oversee quality of care, coordinate case management, and guide staff to achieve program objectives.

You will manage a substantial caseload, collaborate with hospital social workers, and develop community partnerships while ensuring compliance with program standards and reporting requirements.

Qualifications

• Bachelor’s degree in social work or related field.
• Master’s degree in social work or related field and experience with homeless population required.

Responsibilities

• Provide direction and leadership to the social services department.
• Maintain quality guest care by performing quality assurance checks on guest files and sites.
• Manage a caseload of 25+ guests to meet facility needs.
• Coordinate home visits and case management to ensure housing stability for RRH program guests.
• Establish partnerships with community resources to support guest needs.
• Ensure HMIS documentation is accurate and adheres to LAHSA standards.
