Job Summary

Provides psychosocial assessments, plans and implements various counseling, conducts and coordinates discharge planning for complex cases, and participates in the multidisciplinary team process. Victims of domestic violence, adult neglect, abuse, financial fraud, and other DSS issues shall be the Social Worker’s responsibility. The discharge planning service integrates clinical, financial, psychosocial, and legal components, and identifies internal and external barriers that prevent desirable resident outcomes. The Social Worker will provide counseling, education, and resource identification while delivering quality service. Must be knowledgeable of State and Federal laws as they pertain to adults and the elderly. The Social Worker will act as a resource consultant for discharge planning with physicians in their discharge planning role.

Qualifications

• A Bachelor's degree in social work or a human service field is required (Master’s preferred) with 5 years' experience in healthcare or long-term care.
• Extensive experience, preferably in community and long-term care settings.
• Thorough knowledge of social work counseling principles and methods, including guardianship, neglect, abuse, and financial concerns for the elderly.
• Knowledge of state and federal laws related to social work reporting practices.
• Exemplary verbal and written communication skills, creative thinking, and cost-saving practices.
• Proficiency with computers and MS Office software.
