Job Description

The Discharge Planner Case Manager is responsible for managing patient discharge processes, including needs assessments, care coordination, and ordering DME. Requires experience with discharge planning, home health, hospice, LTAC, nursing home, rehab placements, and acute hospital settings. Must hold a valid DC license or equivalent, with BLS certification. Prior travel assignment and regulatory experience preferred. Responsibilities include working within a multidisciplinary team, coordinating benefits, and documenting patient information using Cerner software. Participation in weekend rotations and holiday coverage is required.

Qualifications

• At least 2 years of discharge planning experience
• Valid DC license or equivalent (pending license accepted with proof)
• Basic Life Support (BLS) certification
• Experience with CMS, HIPAA, JCC regulations; workers' compensation preferred
