Job summary

ALIYA Healthcare Consulting seeks a Social Services Discharge Planner to guide residents through transition from long-term care back to the community. You will oversee the Social Services Department's operation, ensuring compassionate care while aligning with regulations.

Responsibilities include policy development, psychosocial assessments, family education, discharge planning, and interdisciplinary collaboration to safeguard resident rights and well-being.

Qualifications

• Bachelor's degree in Social Work or a related field.
• 2+ years of experience in social work or a related field.
• Strong understanding of social work principles, theories, and practices.
• Excellent communication, interpersonal, and organizational skills.
• Ability to work effectively with residents, families, and staff.
• Ability to maintain confidentiality and handle sensitive information.

Responsibilities

• Review facility policies and procedures to ensure compliance with state and federal regulations.
• Participate in setting policies concerning resident care and quality of life.
• Develop facility social work policies and participate in quality assurance interdisciplinary team meetings.
• Document progress in meeting the psychosocial needs of residents and work with the interdisciplinary team to promote and protect resident rights.
• Complete social histories and psychosocial assessments for each resident, identifying their social, emotional, and psychological needs.
• Provide therapeutic interventions to assist residents in coping with their transition and adjustment to a long-term care facility.
• Provide support and education to residents and their families/significant others, referring them to appropriate social service agencies when necessary.
• Coordinate the resident discharge planning process and make referrals for further services.
• Provide clinical interventions to address catastrophic events that occur during a resident's stay
