Job summary

Gilder Search Group is recruiting a Social Services Director for a newly built, high-end skilled nursing facility in Mission Viejo, CA. This role requires hands-on leadership, managing the full scope of SNF social services, and maintaining a robust presence on site to support residents, families, the interdisciplinary team, and admissions partners.

The successful candidate will independently run the department in a setting without dedicated social services support, with full-time onsite

Qualifications

• Proven SNF social services leadership experience, able to run the department independently.
• Experience partnering with admissions and interdisciplinary teams on resident transitions.
• Strong documentation discipline and adherence to regulatory expectations.

Responsibilities

• Lead the social services department and oversee day-to-day psychosocial support for residents and families.
• Complete and maintain psychosocial assessments, care plans, discharge planning and social work records.
• Participate in interdisciplinary care plan meetings with nursing, rehab, dietary and other departments.
• Partner with admissions to support smooth transitions and identify psychosocial needs early.
• Guide residents and families through care transitions, rights concerns, financial needs, and complex family dynamics.
• Coordinate referrals to outside agencies and facility resources as appropriate.
• Support CMS and CA skilled nursing regulations compliance and facility policies.
• Address concerns promptly and maintain clear communication with the Administrator.
• Maintain an organized, responsive social services operation that supports census flow and discharge readiness.
