Bayview Healthcare Ltd. is seeking a Social Services Director to lead the Social Services Department in Brevard County (Melbourne, Viera or Rockledge).

The ideal candidate is energetic, results-driven, and a strong team leader with superior interpersonal and communication skills. You will manage assessments, discharges, MDS processes, and coordinate with vendors and regulators to ensure residents receive high-quality care and rights are protected.

Qualifications

• Experience in Social Services within a Skilled Nursing Facility.
• Bachelor’s degree in Social Work or allied field.
• Understanding of the MDS process and Medicaid-related tasks.
• Ability to coordinate with vendors and regulators.

Responsibilities

• Manage the Social Services Department to meet residents’ needs and priorities.
• Orient residents to long-term care and support placement.
• Complete and document all required MDS for caseload.
• Assess and document hearing, speech, vision, cognitive patterns, mood and behavior; communicate interventions to the care team.
• Maintain professional relationships with vendors such as Hospice, Home Care and DME.
• Ensure compliance with resident rights and Medicaid processes.
