Millennial Healthcare in Brevard County seeks a Social Services Director to lead the Social Services Department in Melbourne, Viera or Rockledge area. The ideal candidate will be energetic, results-driven and possess superior interpersonal and communication skills to guide the team.

The role emphasizes managing assessments, discharges, MDS documentation, Medicaid processes, and collaboration with vendors while upholding resident rights and quality of care.

Qualifications

• Must have at least one year of Social Services experience in a Skilled Nursing Facility.
• Bachelor's degree in Social Work or other allied field is required.

Responsibilities

• Manages department to assure assessments, discharges and psychological needs of residents are met.
• Orient residents to long term care and facilitate adjustment upon placement.
• Complete and document all required MDS for caseload in a timely, accurate manner.
• Assess and document interventions for hearing, speech, vision, cognition, mood, behavior and routines; communicate plan to care team.
• Maintain professional relationships with vendors such as Hospice, Home Care and DME suppliers.
• Understand and promote resident rights; foster positive relations with residents, visitors and regulators.
• Responsible for Medicaid processes and MDS assessments and admissions.
