Job summary

Touchpoints at Chestnut is seeking a full-time Social Services Director to oversee the delivery of Social Work Services for residents, ensuring compliance with CT Public Health Code and regulations. The role directs the facility’s social work services and coordinates with contracted staff.

The director will supervise staff, conduct resident interviews and assessments, assist with discharges, and respond to emergencies and surveys outside standard hours while collaborating with the treatment team.

Qualifications

• MSW preferred; Bachelor’s in Social Work or related field required.
• Minimum 1 year of experience in long-term care delivering direct social services.

Responsibilities

• Supervise the facility's Social Services department and staff.
• Conduct face-to-face resident interviews and assessments of cognitive, mood, behavioral status.
• Assist in planning for resident discharges and care transitions.
• Respond to clinical emergencies, regulatory visits, surveys outside of normal hours.
