Job summary

Touchpoints at Chestnut is seeking a full-time Social Services Director to coordinate and supervise the delivery of social work services for all residents. This role ensures compliance with CT Public Health Code and professional standards.

The director will supervise staff, conduct resident interviews, assist with discharges, respond to emergencies, and collaborate with the interdisciplinary team to develop behavioral and psycho-social programs.

Qualifications

• MSW (preferred).
• Bachelor's in Social Work or related field is required.
• Minimum 1 year experience in long-term care providing direct social services.

Responsibilities

• Supervise the Social Services department and contracted staff.
• Conduct face-to-face resident interviews and psychosocial assessments.
• Assist in planning resident discharges and respond to emergencies outside normal hours.

Education

MSW (preferred)

Bachelor's in Social Work or related field

1+ year LTC social services experience
