Job summary

Bachelor’s in Social Services and healthcare social work experience required; long-term care

Qualifications

• Bachelor’s Degree in Social Services (required).
• Experience in healthcare social work (required).
• Long-term care social work experience is a plus.

Responsibilities

• Provide assessment, advocacy, and discharge planning for psychosocial needs of patients in a long-term care setting.
• Coordinate psychosocial needs with Nursing Department to support resident care.
• Perform crisis intervention and mandated reporting as needed and coordinate care plans for at-risk residents.
• Educate and advocate for patients, families, and significant others to ensure quality care.
