Job Requirements:

• Experience as a social worker within a nursing home or rehabilitation center is strongly preferred.
• Knowledge of rules, regulations, and guidelines governing nursing care facilities.
• MA Social Work License (Required).
• Admission experience: 1 year (Preferred).
• Long-term care experience: 2 years (Preferred).

Licensed Social Worker Responsibilities:

• Plan, organize, develop, and implement social services to meet residents’ needs according to federal, state, and local guidelines.
• Complete all documentation requirements promptly.
• Conduct admission assessments and evaluations.
• Collaborate with the interdisciplinary team to create resident care plans.
• Support residents, families, and staff.
• Attend weekly care plan and Medicare meetings.
• Address and resolve resident grievances.
• Coordinate discharge planning and outside services.
• Participate in monthly team meetings, including QA and Safety.
