• Meets the requirements for the Social Services Director or active social worker license in Kentucky.
• Experience in long-term care or willingness to learn in a skilled nursing setting.
• Advocacy for residents and families within a residential community.

Responsibilities

• Coordinate social services to support psychosocial, spiritual, and emotional well-being of residents and families.
• Oversee daily operations of the Social Service Designee and discharge planning.
• Educate staff on residents' emotional and social needs and assist with Medicaid-related processes.
