Job summary

Pearl Healthcare seeks a Social Services Director to oversee the facility's Social Services Department. The role focuses on planning, coordinating, and evaluating services to meet residents' social and emotional needs in Downers Grove.

The candidate will supervise staff, coordinate discharge planning, and collaborate with families, physicians, and the care team to develop comprehensive care plans ensuring quality, compliant, and person-centered services.

Qualifications

• Bachelor's degree in Social Work or related field (Required).
• Minimum 3 years’ experience in skilled nursing/rehabilitative environment.
• Knowledge of Federal and State regulations.
• Strong interpersonal, verbal, organizational and communication skills.

Responsibilities

• Lead and supervise Social Services Department activities.
• Assess, plan, implement and evaluate patient social needs.
• Coordinate discharge planning and community referrals.
• Maintain relationships with families and residents.
• Communicate with physicians and nursing staff on care plans and discharge plans.
