Job summary

Signature HealthCARE's South Pittsburg Rehab & Wellness Center is seeking a Social Services Director to identify and address residents' social, emotional and psychological needs during their stay and assist in discharge planning.

You will guide care planning, support residents and families, ensure timely documentation in line with state and federal policies, and collaborate across teams to promote person-directed care and quality outcomes.

Qualifications

• Bachelor’s or Master’s Degree in Social Work or Psychology.
• 3+ years of experience in social services, preferably in healthcare.
• High level of professionalism and ability to maintain confidentiality.

Responsibilities

• Assist residents and their families with social, emotional, and family issues.
• Provide timely and accurate completion of documentation needed to comply with Facility, State, and Federal policies.
• Participate in resident care planning.
• Participate in the development of Discharge planning and prepare resident for the transition.
• Serve as a general source of information for residents and their families.

Overview

As the Social Services Director you will identify and provide for each resident’s social, emotional and psychological needs, and the continuing development of the resident’s full potential during his/her stay at the facility and to assist in the planning for his/her discharge.
