Social Services Director
Summary:
As a Social Services Director, you will play a vital role in ensuring the social, emotional, and psychological needs of our residents are met, and assist in the planning for discharge. You will develop social histories, social assessments, and care plans, and implement staff approaches to address problems and needs.
Essential Job Duties and Responsibilities:
- Orient new residents to our long-term/sub-acute care environment and assist with facility adjustment
- Develop and implement individualized care plans, including social histories, social assessments, and goals
- Document progress notes and participate in interdisciplinary Care Plan meetings
- Evaluate resident needs and revise care plans accordingly
- Assist residents and families in discharge and placement planning
- Organize family groups and provide counseling as needed
- Participate in education activities, staff meetings, and training programs
- Promote positive relationships with residents, visitors, and regulators
Other Duties:
You will also be responsible for other duties as assigned, including attending meetings, completing reports, and maintaining accurate records.
Qualifications:
- Currently licensed social worker per State of Nebraska requirements
- Minimum of two (2) years social work experience in a healthcare-related field
- Ability to maintain regular attendance and complete training programs
- Must complete Social Services Certification through The State of Nebraska
Knowledge, Skills, Abilities & Behaviors:
- Proficiency in computer skills and good organizational and time management skills
- Ability to maintain confidentiality and interact professionally with residents, families, and colleagues
- Good verbal and written communication skills
- Ability to relate positively and work cooperatively with all levels of staff
- Ability to meet local health regulations and pass a criminal background investigation
Physical and Sensory Requirements:
The Social Services Director will need to be able to perform the following physical and sensory tasks with or without the aid of mechanical devices:
- Walking/mobility, reaching, bending, lifting, transferring, and repositioning residents
- Grasping, pushing, and pulling
- Fine hand coordination
- Ability to distinguish smells, tastes, and temperatures
- Ability to hear and respond to pages
- Ability to read and write
- Ability to understand and apply training and in-service education
- Ability to instruct other staff in training education and staff meetings
