Job summary

Aventura at the Bay is seeking a compassionate, knowledgeable Social Services Director to support the emotional and psychosocial well-being of our residents.

The Director will advocate for residents and families, work with the interdisciplinary team, and ensure residents receive the necessary resources and services throughout their stay.

Qualifications

• Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or a related field preferred.
• Experience in social services, long-term care, skilled nursing, healthcare, or related setting.
• Knowledge of resident rights, discharge planning, and community resources.
• Strong communication, interpersonal, and problem-solving skills.
• Compassionate and professional approach when working with residents and families.
• Strong organizational and documentation skills.
• Ability to work with an interdisciplinary team.
• Ability to manage multiple priorities in a fast-paced environment.
• Proficiency with Microsoft Office and electronic medical record systems.
• Knowledge of applicable long-term care regulations and requirements preferred.

Responsibilities

• Provide social services and support to residents and families.
• Complete psychosocial assessments and develop individualized social service plans.

• Assist residents and families with adjustment to the facility and changes in care needs.
• Participate in care plan meetings and collaborate with the interdisciplinary team.

• Provide support and guidance to residents experiencing emotional, social, or adjustment concerns.
• Address resident and family concerns and communicate issues to leadership.
• Coordinate with community agencies, healthcare providers, and other resources as needed.
• Assist with admissions, transfers, and transitions of care.
• Help promote resident satisfaction, engagement, and quality of life.
• Maintain confidentiality and HIPAA compliance.
• Participate in facility initiatives, meetings, and quality improvement activities.
