Job summary

Campbell Street seeks a Social Service Director to oversee the Social Service Department, coordinating admissions/discharges and leading an interdisciplinary team to meet residents’ social and emotional needs. You will document care plans, conduct biopsychosocial assessments, and advocate for residents with families and external agencies.

Qualifications

  • Minimum two years’ supervisory experience in a long-term care setting.
  • Must possess required state and federal certifications/licensure.

Responsibilities

  • Plan and direct a comprehensive social services program for residents and families.
  • Coordinate admissions and discharges to maximize census.
  • Participate in an interdisciplinary team to meet residents’ needs.
  • Provide therapeutic intervention for social/psychological aspects of illness and loss.
  • Maintain documentation in medical records per policy and guidelines.
  • Conduct initial biopsychosocial evaluations for residents.
  • Develop social service components of care plans with resident input.
  • Act as liaison between residents, families, outside agencies, and Administrator.
  • Facilitate family group meetings and discharge planning.
  • Manage administrative reporting and budgeting support.
  • Perform additional duties as assigned.

Active cases require ongoing documentation.Develops social service component of the resident’s plan of care identifying specific problems, goals, and approaches. Ensures and documents that residents/families have input into the care planning process.Acts as a liaison/advocate between residents, families, outside agencies, and the Administrator to ensure that the resident’s rights are maintained.Takes responsibility for conducting family group meetings as needed (new admissions group, support group, family and friends’ council, etc.)Performs administrative requirements such as completing necessary forms and reports under the direction of the Administrator and submitting such reports to the Administrator and/or consultants as required.Facilitates discharge planning including the development of an organized discharge plan.Participates in departmental budget planning and supplies monthly report to the Administrator.Perform additional duties as assigned.BENEFITS: At Campbell Street, we believe in taking care of our employees just as they care for others.
