The role focuses on resident well-being, coordination of social work services, and collaboration with medical staff to support high-quality patient care.

Responsibilities include assessments, documentation, and leading interdisciplinary efforts to enhance engagement and quality of life for residents in a long-term care setting.

Qualifications

• Degree in social work.
• Minimum of one year experience in long-term care, skilled nursing, mental health, healthcare, or human services.
• Strong communication, documentation, and resident support skills.
• Ability to work collaboratively with nurses, psychologists, psychiatrists, and healthcare professionals.

• Provide emotional, psychological, rehabilitation, and resident support services under the direction of nursing and medical staff.
• Maintain accurate documentation of patient condition, behavior, eating habits, and daily activities.
• Collaborate with interdisciplinary healthcare teams including nurses, psychologists, and psychiatrists.
• Interview residents upon admission and complete assessments and documentation.
• Participate in recreational and social activities with residents to encourage engagement and quality of life.
