River Bend Nursing and Rehab is seeking a dedicated and compassionate Social Services Director to join our team. This full-time position plays an important role in supporting the social and emotional well-being of our residents while ensuring the effective operation of the Social Services Department.

Key Responsibilities

• Develop comprehensive social assessments and care plans addressing residents’ social and emotional needs, with actionable and realistic goals

• Participate in pre-admission and admission processes to help ensure smooth transitions for residents and families

• Provide compassionate support to residents and families during periods of adjustment, crisis, or special circumstances
