Avir Trilogy in El Paso, TX is seeking a Social Services Director to lead the social services department and address residents' social and emotional needs alongside medical care.

You will conduct assessments, coordinate admissions and transportation, and support transitions home or to another setting. This role requires a degree in social work, Texas licensure, and a compassionate, independent approach.

Qualifications

• Bachelor's or Master's degree in Social Work.
• Texas social work licensure required.
• Two years of experience with geriatric populations.
• Valid driver's license.
• Ability to work independently and exercise sound judgment.
• Patience and a positive attitude with residents, families and staff.
• Ability to move intermittently throughout the workday.

Responsibilities

• Complete social service assessments for every resident and build them into individualized care plans.
• Write quarterly social service progress notes on each resident.
• Handle admissions inquiries by phone and in person, give facility tours, and complete admission contracts.
• Coordinate outpatient appointments and transportation for residents.
• Support residents and families through transfers home or to another facility.
• Connect residents with family members and arrange weekly shopping trips.
• Represent social services at resident council meetings, care plan meetings, and facility committees.
• Produce the bi-monthly resident newsletter.
• Develop and manage the annual social services department budget.

Avir Trilogy is seeking a Social Services Director to join our team!

We're looking for a Social Services Director who knows that the clinical picture is only part of what's going on with a resident. You'll identify social and emotional needs across the building and deliver medically related social services that support each resident's highest practical physical, mental, and psychosocial well-being.

You'll be there at the front end, walking families through tours and admissions, and at every point after, running assessments that shape care plans, coordinating the appointments and transportation that keep life moving, and helping residents transition home or on to their next setting when the time comes.

Qualifications for Social Services Director:
