Cantex Continuing Care Network in Dallas seeks a Director of Social Services to identify and address the medical, social, and emotional needs of patients/residents. The role emphasizes developing social histories, care plans, and coordinating discharge planning.

The position requires a Bachelor's in social work or human services, state licensure or eligibility, and strong communication skills. Prior long-term care experience is preferred and a commitment to regulatory compliance is essential.

Qualifications

Responsibilities

• Develops social histories and a social assessment and care plan for each patient/resident.
• Participates in interdisciplinary care plan conferences and contributes to care plans.
• Coordinates discharge planning with staff, patients/residents, and families.
• Designs and develops social service programs to meet medically related needs and ensures regulatory compliance.
• Interprets and promotes Patients' Rights with the Administrator and leads in-service programs for staff.
• Completes Social Services documentation per state and federal regulations.
• Assists with admissions coordination as needed and may coordinate family council activities.
• Serves as Team Leader/Role Model and participates in committees as requested.

Job Summary:The overall purpose of the Director of Social Services position is to enable the facility to identify the medically related social and emotional needs of the patients/residents. This position acts to meet those needs through individual and group social services programming. This position also promotes patients'/residents potential during their stay and collaborates with physician and patient in preparation for discharge.Qualifications:Minimum of a Bachelors degree in social work or in human services fields.Licensed per the state requirements or eligible for licensure.Skills in communication, documentation, assessment, and social work methods and techniques.Has at least one year of experience in long-term care setting and/or related healthcare setting.Ability to read, write, and speak the English language.Essential Functions:Develops social histories of patients/residents and develops a social assessment and care plan for each patient/resident which identifies medically related social and emotional problems and needs.Participates in interdisciplinary care plan conferences and contributes to the development of care plans for patients/residents.Establishes specific social action plans and realistic goals for each patient consistent with medically related social and emotional needs.Designs and develops social service programs that meet medically related social and emotional needs of patients/residents and complies with regulatory guidelines.With the Administrator, interprets and promotes patients'/residents rights and the \"Patients' Bill of Rights\".Provides individual and group help for patients/residents and families at times of adjustment, crisis, or need.Orients patients/residents and families to the facility environment and assists the adjustment of both through individual and group programs.Completes the Social Services section (sections E & F) of the MDSAssists with implementation of all RCMS and #13 Advanced Directives.May coordinate admissions process for new or returning patients.Coordinates discharge planning with staff, patients/residents, and families.Coordinates social services \"in-service\" and educational programs for staff.Completes all necessary and supportive documentation-progress notes, intervention notes, adjustment notes in accordance with state and federal regulationsMay identify and promote a family council committee.Ability to function as a Team Leader/Role Model.Serves on various committees as requested by Administrator.Has reviewed Cantex Continuing Care Networks Clinical Policies and Procedures or Abuse Prevention and knows the employee's responsibility to enforce it.Responsible for assuring patient/resident safety.Performs other duties/tasks as assigned.#HPWe are an Equal opportunity employer; We offer an excellent benefit plan to include 401K with match, CEU reimbursement, vacation, sick, holidays, medical, dental, and supplemental insurance Plans as well as a Highly competitive compensation package.
