Job summary

Accura HealthCare of Toledo is seeking a Social Services Designee to oversee the Social Services Department and support residents and families from admission to discharge.

The role emphasizes advocacy, education on aging, and collaboration with interdepartmental teams to ensure resident rights and quality care in a skilled nursing setting.

Qualifications

• Post-Secondary education with a bachelor’s degree in Social Work and/or related field preferred.
• Must be able to relate professionally, positively, and cooperatively with residents, families, and employees.
• Must be capable of performing the Essential Functions of this job, with or without reasonable accommodation.

Responsibilities

• Social Services Designees are responsible for the operations of the Social Services Department, providing for each resident’s social, emotional and psychological needs, and the continuing development of the resident’s full potential from admission to discharge.
• Assists in the education regarding aging, Resident Rights, facility services, etc.
• Participates and contributes as a member of interdepartmental teams (IDT, QAPI), following all established policies and procedures.
