Job summary

Ciena Healthcare in Michigan is seeking a Social Services Designee/Discharge Planner to coordinate discharge planning for residents and guests. You will collaborate with the Social Services Department to streamline the process and ensure smooth transitions to the community.

The role emphasizes teamwork, communication, and coordinating with families, practitioners, and the facility to develop individualized discharge plans and arrange post-discharge services.

Qualifications

• High school diploma or GED required.

Responsibilities

• Meet the resident/guest on admission and follow them until discharge to the community.
• Attend the 72 hour care plan meeting.
• Order equipment for the community setting.
• Arrange home health nursing after discharge as needed.
• Coordinate medications for community use.
• Coordinate the entire discharge with IDT involvement.
• Attend care conferences for sub-acute residents/guests.
• Assist in procuring items related to the discharge process for residents/guests returning to the community.
• Initiate and coordinate completion of the Post Discharge Plan of Care.
• Develop an individualized discharge care plan with family and practitioner to ensure needed community care.

Responsibilities may include:

Assist the facility with:

• Meeting the resident/guest on or shortly after the admission and following them until they discharge to the community.
• Attend the 72 hour care plan meeting
• Ordering equipment for the community setting
• Arranging home health nursing for after discharge, as needed
• Coordinating the medications for community use
• Coordinate the entire discharge, with IDT involvement
• Attending care conferences for sub-acute residents/guests
• Assist in procuring any item related to the discharge process for residents/guests anticipated to be returning to the community.
• Initiate and coordinate completion of the Post Discharge Plan of Care
• Develop an individualized discharge care plan for the resident/guest discharge in collaboration with the facility, the family, and the practitioner, assuring the resident/guest receives the care that will meet their needs in the community.
