Job summary

Ciena Healthcare is seeking a Director of Social Services to lead facility staff in residency advocacy, protect residents’ rights, and promote their physical, mental and psychosocial well-being.

Responsibilities include completing admissions, social services evaluations, arranging referrals, coordinating discharge planning, and communicating with residents’ families. Requires a Bachelor’s in Social Work (Master’s preferred) and at least one year of supervised health‑care social work experience.

Qualifications

• Bachelor’s degree in Social Work required; Master’s degree preferred.
• One year supervised social work experience in health care setting preferred.

Responsibilities

• Guides staff in residency advocacy and resident rights protection.
• Completes admissions, evaluations, referrals, and maintains communication with residents and families.
• Assists with personal matters and preplanning referrals (e.g., lawyers, funeral homes).
• Provides discharge planning and coordinates home care services or transfers.
• Provides or arranges counseling services and psychiatric medication management as needed.
• Ensures documentation is timely and accurate in MDS care plans.
• Interviews residents and creates or adjusts care plans as needed.
• Plans, assigns and directs work; prepares work assignment sheets.
• Uses independent judgment on behalf of the organization.

Responsibilities:

The Director of Social Services guides facility staff in matters of residency advocacy, protection and promotion of resident’s rights.

Completes admissions, social services evaluations, provides for social services or referrals, and maintains communication with resident and families.

Assists guests with personal matters (e.g., referrals to lawyers, referrals to funeral homes for preplanning arrangements);

Provides discharge planning services (e.g., helping to place a guest on a waiting list for community congregate living, arranging intake for home care services, assisting with transfer arrangements to other facilities, working with outside agencies toward safe discharges);

Provides or arranges needed counseling services and psychiatric medication management as indicated.

Assures documentation is accurate and timely in MDS care plans.

Interviews residents and creates/adjusts care plans as needed.

Plans, assigns and directs work.

Prepares work assignment sheets.

Uses independent judgment and discretion on behalf of the organization in the performance of these duties.

Qualifications:

Education and/or Experience:

Minimum of Bachelor’s Degree in Social Work. Master’s Degree preferred.

One year supervised social work experience working directly with individuals in a health care setting.
