The Social Services Designee assists with the planning, organizing, implementing, and evaluating social services for residents and patients within the life plan community in accordance with company standards and federal, state, and local standards, guidelines, and regulations to ensure that the emotional and psycho-social needs of the residents and patients are met serving as a link between the underserved communities and existing community resources and health care providers.

Essential Activities and Tasks
Operations Management - 50%
  • May complete admission contract and discuss insurance coverage with the resident/patient and orient them to the campus.=

  • Interviews resident/patient for social service planning while following the schedule of the MDS.

  • Provides residents/patients and families information about and referrals to appropriate agencies and other financial and program assistance as necessary.

  • Counsels residents/patients regarding plans for meeting needs and motivates to maximize inner capacities and environmental resources to improve social function, wellness and health, and prevent avoidable re-hospitalizations.

  • Assists in scheduling of assessments and care plans to be presented and discussed.

  • Participates in discharge planning and coordinating resident/patient level of care changes and manages care transitions communicating vital information for reconnection to community resource agencies and which healthcare teams as needed.

  • Completes advanced directive information with residents/patients as needed.

  • Conducts ongoing counseling, social work intervention, and follow up as needed.

  • Coordinates social services activities with other departments and/or consultants to ensure that daily programming can be performed without interruption.

Care Management and Service Delivery - 30%
  • Listens and responds to the needs of residents, patients and staff members.

  • Reviews and responds to complaints and grievances from residents, patients and staff members regarding all areas of responsibility.

  • Advocates on behalf of the rights of all residents and patients.

  • Assists with resident and patient investigations regarding complaints, incidences, and missing items.

  • Works with residents, patients and staff to resolve issues or acts as a mediator.

  • Provides timely and clear information to others.

Quality and Compliance Management, Administration, and Reporting - 20%
  • Completes a social history on every resident/patient upon admission and reviews with supervisor.

  • Completes necessary forms and reports, including MDS and care plans in the electronic medical records (EMR) system.

  • Ensures that all progress notes and plans of care are informative and descriptive of the services provided and of the resident/patient responses.

  • Maintains the confidentiality of resident and patient information.

  • Assists residents/patients and families with insurance concerns.

  • Willingly and consistently shares observations and insights of departmental operations that may improve quality and efficiencies of department.

  • Attends all-employee and departmental meetings and in-service training as scheduled or as directed.

All other duties as assigned.

Qualifications
Education
  • High school diploma or equivalent required.

  • Associate degree in a related field preferred.

  • If serving in the capacity of Community Health Worker, must have successfully completed the Ohio Board of Nursing Certified Community Health Worker training or be willing to complete within a reasonable timeframe.

Experience
  • Two years experience in a health care and/or human services environment required.

  • Experience in patient advocacy preferred.

  • Experience in a long-term care, hospital, or other related healthcare setting preferred.

  • Knowledge of community resources preferred.

  • Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet required.

Other Requirements
  • Must be able to read, write, speak, and understand the English language.
Working Conditions and Special Requirements
  • Sitting - Up to 8 hours/day

  • Standing - Up to 8 hours/day

  • Walking - Up to 4 hours/day

  • Lifting, pushing, pulling, and moving equipment, supplies, etc. - Up to 25 pounds

  • Work weekends, evenings, and holidays - As needed for coverage

  • On-call availability - 24/7 for emergencies

  • Subject to residents/patients with various disease processes - Occasional

  • May be exposed to infectious waste, disease, conditions, etc. including exposure to the AIDS and Hepatitis B viruses - Work day

  • Risk Category for Exposure to Bloodborne Diseases - II
