Responsibilities

• Monitor patients to detect unusual behavior and report observations to professional staff.
• Provide mental health support and psychosocial care under nursing and medical direction.
• Record and maintain patient condition and activity, including vital signs and behavior.
• Work with psychiatrists, psychologists, psychiatric nurses and social workers as part of the team.
• Assist patients in adapting to hospital routines and activities.
• Encourage participation in social, educational, and recreational activities.
• Administer nursing duties as needed and assist with meals and hygiene tasks.
• Escort patients to appointments, treatments, and activities as required.
• Maintain a safe environment by cleaning and disinfecting rooms and furnishings.
• All other duties as assigned by supervisor or administrator.

Social Services Assistant Requirements:Must have a minimum of one year experience in long term care, mental health, or human services

Social Services Assistant Job Summary: To participate in a multidisciplinary team approach to provide for the psychosocial welfare, serve as a primary advocate, and meet the psychological and mental health needs of each resident to improve or maintain their highest practicable resident physical, mental, and psychosocial well-being. Ensure Federal, State, local, and facility regulations, guidelines and standards are followed.

Social Services Assistant Duties, include but are not limited to:

1. Monitor patients in order to detect unusual behavior, and report observations to professional staff.
2. Provide mentally impaired or emotionally disturbed patients with routine physical, emotional, psychological or rehabilitation care under the direction of nursing and medical staff.
3. Record and maintain records of patient condition and activity, including vital signs, eating habits, and behavior.
4. Work as part of a team that may include psychiatrists, psychologists, psychiatric nurses and social workers.
5. Aid patients in becoming accustomed to hospital routine.
6. Organize, supervise, and encourage patient participation in social, educational, and recreational activities.
7. Perform nursing duties such as administering medications, measuring vital signs, collecting specimens and drawing blood samples.
8. Serve meals, and feed patients needing assistance or persuasion.
9. Restrain or aid patients as necessary to prevent injury.
10. Interview patients upon admission and record information.
11. Provide patients with assistance in bathing, dressing, and grooming, demonstrating these skills as necessary.
12. Participate in recreational activities with patients, including card games, sports, or television viewing.
13. Maintain patients' restrictions to assigned areas.
14. Accompany patients to and from wards for medical and dental treatments, shopping trips, and religious and recreational events.
15. Clean and disinfect rooms and furnishings to maintain a safe and orderly environment
16. All other duties as assigned by the supervisor / Adminstrator.
