Job summary

Parkview Healthcare Center, a member of the CommuniCare Family, is seeking a Social Services Designee to support residents’ psychosocial well-being in a long-term care setting. The role emphasizes advocacy, collaboration with care staff, and timely reporting of resident needs.

Ideal candidates have 1 year of long-term care or behavioral health experience, a high school diploma or GED, and familiarity with resident confidentiality and advocacy.

Qualifications

• 1 year long term care and/or behavioral health experience preferred.
• High school graduate or GED equivalent.
• Previous social services experience in a long term care environment.
• LSW preferred.

Responsibilities

• Develop and maintain a good working rapport with direct caregivers to support programs/services for residents’ psychosocial well-being.
• Report grievances and satisfactory notes to the Director of Social Services.
• Act as resident advocate, coordinating with care staff to meet resident needs.
• Identify changes in residents’ conditions and promptly report to the RN Charge Nurse.
• Provide information to assist solving residents’ problems and improving services.

D. equivalentPrevious social services experience in a long term care environment.Social Worker, LSW PreferredYOUR MISSIONAs Social Services Designee, you will:Develop and maintain a good working rapport with other direct caregivers to assure their participation in and support of all programs/services to benefit the residents’ psychosocial well being.Report all grievances and complaints and make necessary oral/written reports to the Director of Social Services.Serve as the resident’s advocate at all times working in harmony with all direct care giving staff to assure that the resident’s needs are being met at all times (i.e. appropriate dress and clean appearance).
