Job summary

Qualifications

• Bachelor’s degree in Social Work, Non-Profit Management or related field.
• 1–2 years social work experience if BA is in unrelated field.
• Bi-lingual preferred.

Responsibilities

• Conduct psychosocial assessments and program intakes.
• Gather and track client data for programmatic outcomes; ensure charts are documented.
• Maintain updated client paperwork in compliance.
• Communicate with program clients to ensure satisfaction and provide support/referrals.
• Assist clients to access benefits and services; coordinate with medical/mental health providers.
• Provide crisis intervention for housing, medical, mental health, and substance use needs.
• Assist Director of Social Services with staff recruitment, orientation, coaching, and discipline.
• Coordinate scheduling for in-home visits and center-based locations.
• Represent the program at community events and respond to inquiries about services.
• Assist with data collection, monitoring, and reporting for enrollment and social services.
• Plan monthly Social & Activity Groups and supervise interns.

Education
