• Work with residents on discharge and get necessary DME or home care in place so that the residents are safe when they return to the community.

Job Summary

As a Social Services Coordinator at North Ridge Health and Rehabilitation Center, you will play a vital role in ensuring the social and emotional well-being of our residents. You will maintain and improve residents' ability to control everyday physical needs and mental/psychosocial needs, identify individual social and emotional needs, and develop individualized care plans to address these needs. You will also participate in interdisciplinary care plan meetings and work closely with the finance team to ensure residents are educated and receiving proper care.
