Job summary

ChesPenn in Delaware County, PA is seeking a dedicated social services professional to help patients overcome socioeconomic barriers to health and wellness. You will facilitate access to community resources, provide counseling, and coordinate referrals with healthcare teams to support patients and their families.

Minimum two years in social services, an associate degree in social services, and strong communication skills are required; a bachelor’s degree is a plus.

Qualifications

  • Associate degree in social services; bachelor’s degree preferred.
  • Two or more years in social services or related field.
  • Fluency in Spanish is a plus.
  • Knowledge of community resources relevant to social determinants of health.
  • Excellent written and verbal communication; ability to work on a health care team.

Responsibilities

  • Provide individual and family supportive counseling and referrals to improve personal and social functioning.
  • Provide advocacy assistance to obtain necessary services.
  • Partner with Community Health Educator to identify unmet community needs.
  • Facilitate mutual referrals with community agencies.
  • Document all patient contacts and referrals per standards.
  • Maintain confidentiality of patient information and data for reporting.

Position Summary:

Assists the patients of the organization to overcome socioeconomic barriers to achieve their health and wellness goals.Ensures access to quality healthcare for all individuals by facilitating access to community resources, referral services and life skill education for patients and their support systems.

Qualifications:
Education:

Associatedegree in social services,Bachelor’sdegree a plus

Years of Experience:

Two (2) or more years in social services or related field

Knowledge, Skills and Abilities:
  • Knowledge of community resources relevant to social detriments of health
  • Strong verbal communication skills, including excellent active listening skills
  • Excellent written communication skills
  • Ability to work harmoniously as part of a health care team
  • Ability to meet approved minimal physical and medical standards
  • Fluency in Spanish is a plus
Scope of Responsibilities:
  • Provide individual and family supportive counseling and referral to help improve personal and social functioning and resolve emotional and psychological problems
  • Provide advocacyassistancein obtaining necessary services
  • Partners with the Community Health Educator and other agency representatives toidentifyand work to address unmet community needs
  • Partner with community agencies tofacilitate mutual referrals.
  • Provide documentation in a timely manner according to department standards
  • Model skillful, supportive, strengths-based interactions with patients and co-workers
  • Maintain confidentiality of information about patients and their families
  • Perform other duties as directed by supervisor
  • Document all patient contacts, case updates, and referrals according to established protocol and standards
  • Maintain statistical data necessary for reporting requirements
  • Track appointment compliance for care-managed patients who have missed appointments, and document outcomes
  • Prepare reports and audits requested by supervisor
