Townsend Park Health & Rehab in Cartersville, GA is seeking a Social Services Manager to oversee the social services program and lead a compassionate team. You will supervise staff, coordinate family and patient activities, and ensure timely completion of MDS social components.

• Bachelor degree preferred in social work or equivalent supervisory experience in a hospital, skilled nursing center or medical center.
• Experience supervising social services staff is preferred.
• Proficiency with Microsoft Office products and documentation systems.

Responsibilities

• Plan, develop, organize and direct the Social Services Program.
• Coordinate and participate in family and patient activities to promote social interaction and stimulation.
• Counsel patients and families on emotional needs and end-of-life considerations.
• Complete social service portions of the MDS within 14 days of admission.
• Coordinate admissions and discharges; liaise with social, health and community agencies.
• Support Patient and Family Councils and assist with grievance resolution.

Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or associates of the organization.MATHEMATICAL SKILLSAbility to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions, and decimals.REASONING ABILITYAbility to apply common sense understanding to carry out instructions furnished in written, oral or diagram format.
