Job summary

Grace Lutheran Communities in Altoona, WI, is seeking a dedicated Social Services Coordinator to join the River Pines team. In this full-time role, you will develop and maintain social service programs, support residents and families, and collaborate with staff to improve quality of life.

You will oversee admissions, care planning, and advocacy, ensuring compliance with state and federal regulations while upholding the organization’s values of ministry, integrity, teamwork, and excellence.

Qualifications

  • Bachelor’s degree in Social Work or closely related field.
  • Knowledge of Medicare/Medicaid regulations.
  • Ability to follow organization, federal, and state regulations and guidelines, including Resident/Client rights.
  • Strong communication and documentation skills.

Responsibilities

  • Develop, implement and maintain social service objectives, policies, and procedures.
  • Coordinate the Social Services Department and supervise staff.
  • Advocate for residents and investigate complaints.
  • Maintain accurate resident documentation including social histories.
  • Coordinate admissions, relocations, and discharges with community partners.
  • Participate in marketing efforts as a member of the Marketing Team.
  • Plan annual budget for social services.
  • Ensure compliance with state and federal regulations.

ESSENTIAL FUNCTIONS:

  • Develop, implement and maintain social service objectives, policies, and procedures
  • Foster positive communication within the organization as a whole; promote organizations’ team concept
  • Interprets philosophy, objectives, policies and procedures of organization to social services personnel
  • Make decisions and perform all duties in accordance with organization’s policies/procedures, state and federal regulations and in conformance with recognized standards
  • Coordinate the Social Services Department; develop and implement quality social services to resident and families; empowering staff to be creative, offer input into the decision making process and to develop program.
  • Maintain contact with community discharge planners, coordinate resident admissions, relocation, and room transfers
  • Coordinate community placement/needs and discharges according to state and federal regulation and organization policies
  • Serve as resident advocate for the facility, resident and family, paying particular attention to resident rights; serve as complaint investigator
  • Maintain concise and accurate resident documentation, including social histories
  • Review assessments, care plans quarterly reviews, progress notes, MDS and discharge plans
  • Provide individual/group counseling for residents and their families relative to personal needs, social relationships, personal and financial needs and individual concerns
  • Ensure fair and consistent treatment of staff; continually monitors and evaluates staff performance; Investigates and initiates incidents of discipline/corrective action (including discharge), if necessary, according to established personnel procedures
  • Accessible to staff on all shifts
  • Plan annual budget and works within an approved budget; supervises and proves all social services purchases
  • Plan and implement a social services development program, including required in-service education, to meet the needs of all social services staff
  • Maintain and safeguard confidential information
  • Communicate effectively with residents, families, co-workers and public
  • Function independently in situations that require professional judgment
  • Serve as a role model by recognizing and meeting the needs of the residents, families and staff through effective use of "Safety, Courtesy, Care and Efficiency" standards
  • Coordinate care conference scheduling
  • Participate in Marketing of the facility; takes an active role in Marketing as a member of Marketing Team
  • Ensure compliance of Social Services department with State and Federal regulations
  • Participate in the Quality Assurance/Improvement Program

PHYSICAL REQUIREMENTS:

  • Frequent twisting; occasional bending
  • Reaches at shoulder height frequently; reaches above or below shoulder height occasionally
  • Lifts and carries less than 5#
  • 85% of work day is spent sitting; 10% walking, 5% standing

OTHER REQUIREMENTS:

  • Bachelor’s degree in Social Work from an accredited college and certified or eligible for certification as a Social Worker from Wisconsin Department of Regulation and Licensing or has a related degree and experience in Social Services and will receive at least monthly counseling from a certified social workers
  • Knowledge of and experience working with Medicare/Medicaid regulations
  • Follows/adheres to organization, federal, and state regulations and guidelines, including Resident/Client rights
  • Maintains neat, well-groomed appearance, adhering to dress code of organization/program
