Job summary

Esperanza Health Center seeks a bilingual Social Service Consultant to join our interdisciplinary care team in Philadelphia. You will assess social determinants of health, connect patients to community resources, and reduce barriers to care.

Work with medical providers and health staff to deliver patient-centered care in a culturally responsive, faith-based ministry. This is a full-time role with a fixed schedule and opportunities for growth.

Qualifications

• Bachelor’s degree preferred.
• Bilingual in English and Spanish required.
• Ability to work effectively in a fast-paced, team-based environment.
• Strong communication, organization, and interpersonal skills.
• Commitment to a Christian healthcare ministry.

Responsibilities

• Meet with patients in person or by phone to assess social determinants and provide resources.
• Maintain accurate documentation in the EMR.
• Provide limited case management for patients living with HIV.
• Offer crisis intervention and referrals for domestic violence or child abuse.
• Conduct HIV counseling and testing per guidelines.
• Assist uninsured patients with enrollment in health insurance programs.
• Support insurance renewal processes as needed.
• Facilitate referrals to Early Intervention and Autism Evaluation services.
• Assist families in navigating education, welfare, and external agencies.

• High School Diploma: $18.00 to $20.50 per hour
• Bachelor’s Degree: $20.25 to $23.25 per hour

Position Details

• Department: Social Services
• Reports To: Social Service Supervisor
• Status: Full-Time, Non-Exempt
• Schedule: Monday–Friday, 40 hours/week
• Travel: Less than 10%

Position Summary

Esperanza Health Center is seeking a bilingual Social Service Consultant (SSC) to join our interdisciplinary care team. Social Service Consultants work directly with patients—both in person and by phone—to assess social needs, connect patients to critical community resources, and reduce barriers to care.

SSCs collaborate closely with medical providers, behavioral health clinicians, and community health staff to deliver comprehensive, patient-centered care addressing social determinants of health in a culturally responsive and holistic manner.

Key Responsibilities

• Meet with patients in person or by phone to assess social determinants of health and provide culturally appropriate resources
• Maintain accurate documentation in the Electronic Medical Record (EMR)
• Provide limited case management services for an assigned panel of patients living with HIV
• Provide crisis intervention and referrals for patients experiencing domestic violence or child abuse
• Conduct HIV counseling and testing in accordance with established guidelines
• Assist uninsured patients with enrollment in health insurance programs and follow up to address barriers
• Support patients with insurance renewal processes as needed
• Facilitate referrals to Early Intervention and Autism Evaluation services
• Work with children and their families to support linkage to needed resources and assist with navigating the educational system, welfare system, and other external agencies impacting health and well-being
