Job summary

Esperanza Health Center, Inc. is seeking an Intensive Care Manager to join our Behavioral Health Services team in Philadelphia.

This 40-hour, full-time exempt role supports patients with substance use through referral, care management, and adherence support, including MAT, within a trauma-informed, culturally sensitive framework. You will collaborate with clinicians, coordinate individualized care plans, connect patients to community resources, monitor progress, and document care in our EMR.

Qualifications

  • MUST meet one of the following requirements: Master's degree in Social Work, Counseling, Psychology, or related field working towards LSW licensure.
  • OR Bachelor’s degree and enrollment in a qualifying Master’s program for licensure.
  • Candidates pursuing clinical licensure (LCSW, LPC, or LMFT) are strongly preferred.

Responsibilities

  • Receive and manage patient referrals; collaborate with staff to facilitate initial screening and intake.
  • Provide ongoing support and care management for individuals in recovery or with substance use, using a culturally sensitive, trauma-informed approach.
  • Develop individualized care management plans addressing identified needs and patient goals.
  • Identify and connect patients to culturally appropriate resources; provide external referrals as needed.
  • Monitor recovery progression; communicate progress to the care team and flag concerns.
  • Amend care plans in collaboration with patients and teams; match patients to appropriate levels of care.
  • Provide crisis intervention, crisis management, and safety planning as appropriate.
  • Document all progress and services in the EMR in a timely manner.

REQUIRED QUALIFICATIONS

JOB SUMMARY

The Intensive Care Manager (ICM) works alongside our medical staff and licensed behavioral health

professionals to provide comprehensive case management services to patients seeking support with substance

use. Services include referral and linkage, individualized care management, and support for adherence to

available treatments, including Medication Assisted Treatment (MAT). The ICM practices within a Substance Use

Prevention and Harm Reduction (SUPHR) framework using culturally sensitive, strength-based, and trauma-

informed approaches.

WHAT YOU WILL BE DOING
  • Receive and manage patient referrals; collaborate with departmental staff to facilitate and expedite initial screening and intake.
  • Provide ongoing support and care management to individuals engaged in substance use, utilizing a culturally sensitive, strength-based, trauma-informed approach to build and maintain a therapeutic alliance.
  • Collaborate with patients and care teams to develop individualized care management plans that address identified needs and support progress toward patient-identified goals.
  • Identify and connect patients to culturally appropriate resources across EHC and the broader community; provide referrals to outside agencies as appropriate.
  • Monitor and evaluate each patient's recovery progression; communicate progress to the care team regularly and flag concerns as they arise.
  • Address and amend care plans as needed in collaboration with patients and care teams, utilizing clinical assessment skills to match patients with the appropriate level of care, including referrals to higher levels of behavioral health care when indicated.
  • Provide crisis intervention, crisis management, and safety planning as appropriate.
  • Document all patient progress and services provided accurately and in a timely manner in the Electronic Medical Record (EMR).
PHYSICAL DEMANDS AND WORK ENVIRONMENT
  • Ability to sit, stand, and move throughout the clinic for extended periods
  • Ability to use a computer and telephone for extended periods
  • Occasional travel between Esperanza sites as operationally needed (less than 10%)
  • Ability to remain composed and professionally effective in high-stress situations, including crisis
  • Ability to lift and carry materials up to 15 lbs. as needed
