Job summary

The VIOS Clinic seeks a compassionate IRIS Consultant to support adults, older adults, and individuals with disabilities under Wisconsin’s IRIS program. You will build trusted relationships, educate on eligibility, and collaborate with community providers to promote independence and inclusion.

Responsibilities include maintaining documentation, supporting annual screens, and working within an interdisciplinary team to monitor outcomes and adapt plans to participant needs.

Qualifications

• Bachelor’s degree in Social Work or related field.
• Experience or certification relevant to IRIS is preferred.

Job description

We are seeking a compassionate and organized IRIS Consultant to support adults, older adults, and individuals with disabilities through Wisconsin's IRIS (Include, Respect, I Self-Direct) Program.

Key Responsibilities Include

• Building trusted relationships with participants and assisting in creating and updating individualized support plans
• Promoting participant choice, self-direction, independence, and community inclusion
• Monitoring health, safety, and program outcomes
• Educating participants about IRIS eligibility, resources, and enrollment requirements
• Collaborating with community organizations, healthcare providers, and service agencies
• Maintaining accurate case documentation and completing required compliance reports
• Supporting annual functional screens and working independently within an interdisciplinary team

Qualifications Include

• Bachelor’s degree in Social Work, Human Services, or related fields with at least one year of supervised experience working with older adults or individuals with disabilities,
• Or four or more years of direct service experience,
• Or current IRIS Consultant in good standing with a Wisconsin DHS-approved exemption.

Preferred skills and experience

case management, care coordination, disability and long-term care services, social work, Medicaid programs, behavioral health, and person-centered planning. Strong communication, cultural awareness, proficiency with technology, and ability to work independently are essential. Bilingual candidates in languages such as Spanish, Hmong, Arabic, Russian, or ASL are encouraged to apply.
