The Social Services Case Manager (SSCM) works as part of a team providing case management, training and support services, transportation, public education, and the facilitation of support/educational groups for VisionCorps clients while acting in accordance with company values, vision, and mission.
Education, Experience, & Skills
- Bachelor degree in Human Services, Social Work, Psychology or related field
- Ability to successfully secure clean clearances for PA criminal history, child abuse, and FBI fingerprinting
- Proficient computer skills with the ability to use Microsoft Office Suite and other cloud products
- Proficient written and oral communication skills with excellent listening skills
- Two (2) years of professional experience working with individuals with vision loss or another disability in a setting like rehabilitation centers, schools or community services preferred

Key Duties and Responsibilities:
- Facilitate new client intakes, including but not limited to assessing client needs, strengths, goals and support networks
- Provide Training and Support Services (TSS) as defined in the grant through the Pennsylvania Association for the Blind including, but not limited to: transportation/escort, life skills, and support services to eligible clients
- Facilitate proper referrals to other VisionCorps providers and external agencies as appropriate
- Plans and implements client educational & support groups as assigned
- Maintain appropriate documentation of all client contact and case management services provided in CRM, including TSS records relating to eligibility, service plans, progress, and change in status
- Collaborate as part of a team to serve clients throughout the organization's multi-county service area
- Provide public education on VisionCorps services & low vision/blindness issues
- Keeps up to date on advances in the field of vision loss, rehabilitation, and mental health through training, seminars, and other educational avenues
- Manage volunteers and support staff in the delivery of services as appropriate/assigned
- Build and maintain collaborative relationships with local and regional organizations for the benefit of clients, VisionCorps, and the surrounding community
- Other duties as assigned
Physical Demands:
- Ability to work a minimum of full-time hours; overtime required based on Rehabilitation department schedules
- Fine motor skills – use hands to finger, handle or feel
- Occasionally required to stand; walk; use hands to touch, handle and/or operate objects, tools, or controls; reach with hands and arms
- Occasionally required to sit for extended periods of time which may include vehicle
- Occasionally lift and/or move objects up to 30 pounds
- Provide services in office, virtually, or in client's natural environment (home, retirement community, assisted living, skilled care, etc.)
- Travel up to 70% of the time, which may also include public-facing forums and company-sponsored events
- Availability of a personal vehicle for travel when an agency vehicle is not available
