Job summary

Southern HealthCare Management is seeking a Social Services Assistant to support the social service function in a long-term care and short-term rehabilitation nursing center in Jacksonville, FL. You will assist with admissions, psychosocial assessments, care plans, and discharge planning to ensure a comfortable and positive experience for residents and patients.

Qualifications

• Bachelor's degree in social work or related field required.
• One year supervised social work experience in a health care setting.
• Experience with patient registration, care coordination, discharge planning or related social service/healthcare office work.
• Must be familiar with medical terminology.

Responsibilities

• Welcome and admit new residents and patients.
• Complete psychosocial assessments and care plans.
• Complete discharge plans and related documentation.
• Support the social services function in collaboration with the Social Services Manager.

As a Social Services Assistant, you will support the social service function in a long-term care and short-term rehabilitation nursing center. You will work closely with the Social Services Manager to welcome and admit new residents and patients, complete documents related psychosocial assessments, care plans, and discharge plans, and ensure a comfortable and overall positive experience is achieved while completing treatment plans. Qualifications

• Minimum high school diploma or equivalent required.
• Minimum of a bachelor's degree in social work or a bachelor's degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology and
• One year supervised social work experience working directly with individuals in a health care setting.
• Minimum of a bachelor's degree in social work or a bachelor's degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology and
• One year supervised social work experience working directly with individuals in a health care setting.
• One or more years' experience in health care education/ marketing / insurance or commensurate educations preferred.
• Must be familiar with medical terminology.
• Proficient in the use of personal computer.
